Individual Managed Futures accounts will need to complete: the appropriate account application form, CTA Disclosure Document, appropriate POA and any others forms unique to your account. You have three choices for your account application: download and mail, apply online or fax.

Commodity Pool participants will only need to download and complete  the Commodity Pool Disclosure Document.
